All you need to know about Ear Infections

Ear infections are the second most common illness in childhood after colds. About half of babies have at least one ear infection by the time they turn one year old, and most have had at least one ear infection by their third birthday.

An ear infection can be caused by bacteria or a virus. It happens when fluid builds up in the area behind your child’s eardrum and then becomes infected. Normally, any fluid that enters this area leaves quickly through the eustachian tube, but if it’s blocked – as often happens during colds or sinus infections – the fluid gets trapped in the middle ear and becomes a perfect breeding ground for germs.

Children are more likely than adults to get ear infections because they have short, horizontal eustachian tubes. As children grow to adulthood, their tubes triple in length and become more vertical, so fluid can drain more easily.

The onset of signs and symptoms of ear infection is usually rapid. In children, these may include:

  • Ear pain, especially when lying down
  • Tugging or pulling at an ear
  • Fever of 38 C or higher
  • Difficulty sleeping
  • Fussiness
  • Loss of appetite
  • Difficulty hearing or responding to sounds
  • Drainage of fluid from the ear
  • Headache

When to see a Doctor

Call your child’s doctor if:

  • Symptoms last for more than a day
  • Ear pain is severe
  • Your infant or toddler is sleepless or irritable after a cold or other upper respiratory infection
  • You observe a discharge of fluid, pus or bloody discharge from the ear

Doctors use an otoscope, a small instrument similar to a flashlight, through which they can see the eardrum. After which they will decide on the next steps taking into consideration the following:

  • The type and severity of the ear infection
  • How often your child has ear infections
  • How long this infection has lasted
  • Your child’s age
  • Risk factors your child may have
  • Whether the infection affects your child’s hearing

Tips to Reduce the Risk of Developing Ear Infections:

  • Prevent common colds and other illnesses.

Teach your children to wash their hands frequently and thoroughly and not to share eating and drinking utensils. If possible, limit the time your child spends in group child-care. Try to keep your child home from child-care or school when ill.

  • Avoid secondhand smoke.

Exposure to tobacco smoke or high levels of air pollution can increase the risk of ear infection.

  • Breast-feed your baby.

If possible, breast-feed your baby for at least six months. Breast milk contains antibodies that may offer protection from ear infections.

  • Talk to your doctor about vaccinations.

Seasonal flu shots and pneumococcal vaccines may help prevent ear infections.

Getting the Facts from Dr. Amin Sharobim

Who can resist the holiday combination of summer, sea and sand? Each year millions of holiday-makers travel thousands of miles to bask in radiant sunshine, chasing the perfect tan. So, what’s the catch? Too much sun exposure can create lasting skin damage and has associated health risks. Cairo West Magazine learns more about protecting our largest organ with leading dermatologist Dr. Amin Sharobim.

CWM: Dr. Amin, Is there a difference in the type of sun-block protection that men and  women need?

There are important common factors to bear in mind for all family members. It is important to use a protective sunscreen that suits your own skin type – oily or dry – and sensitivity level. There is no essential difference for products that are suited to men or women if you follow those guidelines. Anyone with dry skin needs to choose a sunscreen with a high level of moisturizer, whether lotion or cream. In addition to an SPF of 30+, your sunscreen should include some combination of the following UVA-blocking ingredients: zinc oxide, titanium dioxide, avobenzone, ecamsule, & oxybenzone.  Sunscreens with both UVA and UVB protection may be labelled multi spectrum, broad spectrum, or UVA/UVB protection. For best protection, we recommend using a minimum SPF sunscreen of 15, applying the proper amount (2mg/cm2 of skin, or about one ounce for full body coverage) at least 20 minutes before direct sun exposure, and reapply every 2 hours.

What are specific protection guidelines for very young children?

Children, of course, have delicate skins that are in need of a sun-block with a factor of SP30+ up to SP 50+, while containing a minimal amount of chemicals. These tend to contain natural products, and are put on in a layer that physically coats the skin and deflects the sun’s rays. It is important when choosing a product to try it on a small patch of skin and leave for at least half an hour to see if there is any adverse reaction before applying it all over. Look for broad spectrum protection from UVA and UVB rays using the safe mineral zinc oxide, avoiding oxybenzone, octinoxate, or other chemical active sun-screening agents. Very young children should ideally be kept in the shade and covered with loose protective clothing.

What do we need to know about the harmful effects of over exposure to direct sunlight?

UV radiation reaches the earth in the form of UVB and UVA rays.  UVB radiation plays a key role in skin cancer and is the chief culprit behind sunburn, while UVA rays, which penetrate the skin more deeply, are associated with wrinkling, leathering, sagging, and other light-induced effects of aging (photo-aging). They also exacerbate the carcinogenic effects of UVB rays, and increasingly are being seen as a cause of skin cancer on their own. Sunscreens vary in their ability to protect against UVA and UVB. SPF refers mainly to the amount of UVB protection a sunscreen offers.  Thus, higher SPFs can help: An SPF 15 sunscreen blocks 93 percent of UVB radiation, while an SPF 30 sunscreen blocks nearly 97 percent.  Above SPF 50 (which blocks an estimated 98 percent of UVB rays), the increase in UVB protection is minimal.  Furthermore, higher SPF values offer some safety margin, since consumers generally do not apply enough sunscreen.  To evaluate SPFs, testers apply two milligrams of sunscreen per square cm of skin.  But in everyday life, most people apply from only 0.5 to one milligram per square cm of skin.  Consequently, the actual SPF they achieve is approximately 1/3 of the labelled value. Sunscreens with a higher SPF should offer more protection from the sun’s harmful ultraviolet (UV) radiation, which is linked to the vast majority of skin cancers, as well as premature skin aging and eye damage.

How important a role does natural skin tone play in the ability to deal with UV rays?

Here in Egypt, like many of our Mediterranean neighbours, we are fortunate generally to have a higher level of melanin in our skin, giving most of us a naturally olive complexion. This helps us cope with the constant sunshine and high temperatures, making us less likely to burn when exposed. This does not mean we can lie around in the sun with impunity, but we are less likely to burn than fairer-skinned visitors.

A lot of people are expressing concern about ingredients in commercial sun protection products, what are the chemicals to avoid?

There can be severe allergic reactions to products containing Para-aminobenzoic acid, so it may be best to check the label carefully before applying. Effects can include coma, diarrhea, Dizziness, eye irritation if it touches the eyes, fever, liver failure, nausea, rash (allergic), shortness of breath, slowed breathing, stupor (altered thinking and decreased level of consciousness) and vomiting. Many of the chemicals in sunscreens may create allergies, so again, please carry out a small skin test first.

There is a trend towards natural products for tanning. What is your opinion of these: i.e. coconut oil?

Dermatologists will firmly advise patients to avoid any form of tanning, including the use of sun-beds. The use of any oil, no matter how organic, in helping you fry yourself, is putting your skin and health at risk.

What is the best remedy for sunburns?

When you have suffered from over-exposure, go for a good moisturiser with high levels of anti-oxidants. Many of these contain aloe vera, which is known for its’ soothing properties.

Apart from topical products, what are the top tips you can give on avoiding excess exposure to the sun?

Wear loose light-colored clothing in natural fabrics like cotton and linen. Wear a hat, and remember that even on a cloudy day you are catching UV rays.

What are the best times of day to be out in direct sunlight?

It is important to avoid the hours between 11 am and 4 pm. The sun is at its strongest between those hours, and you can burn very quickly. Try to enjoy the beach earlier in the morning (when it s often less crowded), or in the late afternoon.

Are there any nutritional items that can assist our skin in keeping healthy in summer?

Load up on fresh seasonal fruits and vegetables, they will keep you hydrated and give you the antioxidants you need to help your skin repair itself. In recent tests certain nutrients, such as the antioxidant vitamins C and E, have been found to help prevent, and repair cellular damage caused by ultraviolet radiation. Eating foods that are high in these vitamins on a day that you’ll be out in the sun might actually prevent or lessen the severity of a sunburn! You get the best protection if you have both of these vitamins in your system at the same time, but they are hardly ever found in the same foods. You get vitamin C from tomatoes, red and green peppers, melons, citrus fruit, and broccoli. Vitamin E, on the other hand, is found in avocados, almonds, and sunflower seeds.

 

Why are men turning to cosmetic surgery

Summer at the beach can be unforgiving at times. This is when months of winter hibernation, too many hours in front of the TV, and lots of comfort food show on our lightly covered beach-bare bods. Traditionally considered a female domain, body-sculpting procedures are now becoming a hot trend with men. While there is no disputing that a healthy diet and good exercise routine are optimum for a toned physique, a little cosmetic help can also go a long way. Cairo West Magazine sat down with leading plastic surgeon Dr. Hani Nabil, to learn what is available and what results can be expected.

 

CWM: Dr. Hani, which are the most frequently requested body-sculpting procedures for men?

Generally we see a high demand for procedures targeting specific deposits of excess body fat. Excess fat around the upper hip area, or ‘love handles’ are a common problem. We also deal with cases of gynecomastia, which is the removal of excess fat in the breast area. Of course, our 6-pack procedure is extremely popular, as it trims away fat to reveal the natural muscle structure underneath. We handle reshaping of double chins and belly reductions as well.

What techniques are used?

We utilize different levels of liposuction and lipo-sculpture to remove stubborn localized areas of fat. The technology selected depends on the individual case, and amount of fat that is to be removed. It could involve Vaser® ultrasound technology, which stands out in its ability to differentiate targeted fat from other important tissues – such as nerves, blood vessels and connective tissue, giving a superb degree of precision, rapid healing, and great results.  The latest state-of the-art PowerX® System is a power-assisted liposuction device. It is built for speed and rotates based on predetermined angle and speed settings, allowing us to quickly remove fatty tissue all the way around the cannula. Depending on the case requirements we might alternatively use Smartlipo® laser technology or Moller’s Vibrasate® vibration technology. These procedures are all quick and have minimal downtime for recovery.

So, how long does it generally take and how much downtime is generally required after each procedure?

We can carry out a session in around 60 minutes. There may be a slight discomfort for around 24 – 48 hours, with light bruising for up to ten days. That makes it possible to have a 6-pack procedure in time to hit the beach with family and friends.

Are any of these procedures non-invasive?

The aforementioned are defined as minimally invasive procedures. For cases where only 2-3cm of stubborn fat need to be removed we might recommend cryolipolysis, or Cool- Sculpting. In this procedure fat cells are targeted and frozen to the point that they are broken down. The body can then dispose of them through its normal processes. This is a non-invasive and highly effective technique, which targets fat cells without damage to other cells and tissue. This program does not require anesthesia, leaves no scars, and does not require any post treatment. Ninety-two percent of patients who had just one CoolSculpting treatment had a noticeable reduction of fat in the treated area

What benefits would a man experience from these procedures?

Apart from the obvious cosmetic ones, such as a trimmer physique and defined muscular structure, the removal of excess adipose fat also can reduce the risk of weight related medical incidents like stroke and heart attacks. While a procedure can make an immediate and noticeable difference we also encourage patients to support this with a sensible long term nutrition and exercise plan to help them maintain their shape. Overall the boost in self-confidence and self-esteem can be life changing.

Is there an optimum season of the year for carrying out a procedure?

Not really, although anyone with sensitivity to heat may feel more comfortable doing this in cooler months as they may need to wear an elasticized support garment for a few days.

Are there any pre-requisites for undertaking these procedures?

Most men who use them are over 40. This is when a sedentary office-bound lifestyle is likely to have affected their shape. It is important to have a certain level of strength in the muscles, and elasticity in the skin to be able to have a good result. We carry out a clear assessment in our consultation with our clients prior to any procedure.

When should this type of cosmetic procedure be avoided?

There is a contra-indication for heart patients using stents and anyone in poor health generally. We have no problems with diabetes cases so long as the medication used is under the correct control guidelines.

How long do results last?

The results are stable so long as a person gains no more than 20% of their body weight. That is why a healthy lifestyle plays such a role in maintaining the new shape and body tone.

Apart from these targeted weight reduction and body sculpting techniques, what else can men consider?

Men, like women, are often chasing that elusive fountain of youth. They may not be so willing to admit it though! We have a wide selection of age-reducing therapies and procedures, ranging from skin rejuvenation and 30-minute eyelid lifts to surgical and non-surgical chin augmentation.  There are even ‘scar-less’ facelifts, which give amazing results and take very little time. The great thing is that they give a really fresh youthful appearance with no virtually no time away from a patient’s normal daily schedule.

Can procedures be repeated after a certain period?

The good thing is that there really shouldn’t be any need. Of course, there is no reason they cannot be carried out, should it be requested.

Galleria 40 is taking very calculated steps on its way to becoming the go-to venue for the epicurean gourmands in all of us, Cachimba is a Spanish cuisine eatery with authentic Spanish delights cooked up by Chef Cristobal Angulo.

The indoor seating area offers a direct view onto the outdoor area and beyond through the floor-to-ceiling glass walls. With the first ever shisha bar in town, shisha-lovers can get a front row seat to the making of their shisha in a variety of exciting mixes.

What greeted us upon being seated by the friendly staff was a refreshing carafe of the house sangria made with fresh juice, and freshly cut fruit. Following our sangria we were treated to a traditional gazpacho Andaluz, a cold tomato soup, packed with everything from fresh veggies, to spices like cumin, and even bread.

What’s Spanish cuisine without tapas? We began our foray into cold tapas with the montaditos de ensaladilla de atun, a velvety tuna and potato concoction on a soft round slice of the house saffron bread. Following that was the montaditos de quest fresco con olivas y langostino, a creamy olive-cheese with shrimp, and succulent pepper marmalade on saffron bread.

Sipping on our rich sangria, conjuring images of poolside leisure… our daydream was immediately interrupted by a slew of warm tapas. We began with a house specialty; one of the dishes Cachimba is best known for, the rabas de calamari, a dish of perfectly golden crispy fried calamari strips. Then down came the croquets con chorizo, cute little chicken croquettes served with slices of the most punchy, tangy chorizo. Accompanying this dish was empanadillas de ternera; a pastry filled with spicy beef and vegetables, and the incredibly scrumptious gambas al ajillo, perfectly cooked shrimp in a mouth-watering garlic sauce.

Then the star of the evening appeared before our eyes, paella de frutos del mar, by far the richest, most authentic incarnation of Spanish paella in Egypt. Big enough to feed a party of four (we were only 2), we devoured the jumbo shrimp, the lobster tails, the seafood goodness tucked within the soft yellow mounds of rice… it was more than our pleasure to lick that dish clean.

With literally no room left to breathe, we shared a sorbete de limon for dessert, a refreshing palate cleanser of lemon sorbet with mint, tonic and tiny chewy pieces of lemon skins… the perfect ending to one of the most perfect dinners you will ever have in West Cairo!

Cachimba also offers a variety of cocktails, juices, lemonades and iced teas, hot drinks including some Spanish coffee delights, and milk shakes. A Ramadan menu is also available. With a branch currently open in Galleria 40, Cachimba is set to open its second branch at Cairo Festival City this July.

Last month, we previewed our spectacular new beverages to select members of the media at our Cairo Festival City branch. Featuring a wide collection of smoothies, mocktails, coffee shakes and juices with a twist, the new beverages menu contains over 40 drinks to suit everyone’s tastes. Each drink has been mixed, named and presented with extreme attention to detail; from the carafes, glasses and drinking jars used all the way to cherries and cotton candy garnish.

Knowing how to make the perfect smoothie is not as easy as you think but it’s not impossible either. There’s no better way than to learn how to make your own smoothie at home with the ingredients you desire, making sure it’s only packed with natural sugars if any.

Follow these simple steps to help you obtain a healthy and refreshing smoothie:

1) First, the base. You need to have 1-2 cups of liquid as your base to set the way for your smoothie. It all depends on your caloric intake whether you want to cut down the sugar intake or not. Some of the healthy and low fat options include water, soy/coconut milk or yoghurt. Other creamier and richer options include full cream milk or fruit juice.

2) Second, you add the fruit/vegetable preferred. Among the most popular ones and highly recommended are bananas, strawberries, peaches, and mangoes.

3) Third, give your smoothie that special kick by adding a special extra ingredient that will add both density and flavor to your smoothie. Some of those special extras include: honey, cinnamon, brown sugar, vanilla extracts, ice cream, peanut butter, oats and many more.

4) Fourth, the ice. If you have more fresh fruit/vegetables than frozen ones, you will need to add more ice to give it a cooler temperature and thicker texture (5 – 6 cubes of ice should do but that depends on your preference of how cold you want it to be). If you have more frozen fruits/vegetables, you won’t need as much ice (1 – 2 cubes of ice should do).
